Annual Release Forms

It is that time of year again. Below is a link that will open the release form for 2009-2010. If you fill out this one form you will be set until Agust 2010. The only other release forms you would need to fill out in the next year would be forms that specific locations ask for. Please click the link below, print the file, fill it out and return it to the church office as soon as possible. Thanks
